Muse Healthcare Insights for Better Patient Care

Hospice care focuses on improving a patient’s quality of life and ensuring their end-of-life experience is as peaceful as possible. Ultimately, the hope is that patients maintain dignity and that no one transitions alone.

According to a recent JAMA study, 12.3% of patients on routine hospice care in the home received no skilled visits in the last two days of life.

To help address this issue, Agape Care became the first hospice care provider in South Carolina to adopt predictive modeling technology from Muse. Leveraging Muse technology, Agape Care may more accurately predict end-of-life transitions to ensure the right level of care for patients in the last seven to 12 days of life.

HOW IT WORKS:

  • Muse considers more than 800 assessment points from patient visit data to assist with determining more accurate end-of-life transitions
  • Muse categorizes and emphasizes patients according to their needs, then it alerts clinical supervisors when a patient would benefit from additional scheduled skilled visits
  • Muse’s critical patient rankings have proven to be 92 percent accurate in determining a transition in seven days or less

When hospice care providers can more accurately predict when their patients will transition, they can ensure their patients and their patients’ families receive the care that matters most in the final days and final hours of a patient’s life.
